The Wasatch Kids Loyalty Program is our in-house dental membership program. Here at Wasatch Pediatric Dentistry, we strive to provide our patients with the highest standard of excellence in pediatric dentistry in Logan, Providence, and Cache Valley. We understand that a lot of families do not have dental insurance. This is why we offer our in-house dental membership program. Waiting until there is an emergency will always end up being more expensive in the long run. With our discount plan, you will be able to enjoy the benefits of regular, preventative dental visits to help keep your child’s smile healthy and affordable for every family in Providence, Logan, and Cache Valley, UT.

Unlike a dental insurance plan, there are no third-parties, no deductibles, no co-pays or waiting periods. Enrolling in our Wasatch Kids Loyalty Program gives you the opportunity to obtain your dental treatment exclusively at our practice for a reduced price.
Membership Benefits Effective January 2023
CHILDREN 0-2: $230/yr
CHILDREN 3-12: $400/yr
CHILDREN 13+: $500/yr
Membership Benefits Include:
2 doctor exams per year
2 professional cleanings per year
2 fluoride treatments per year
Bitewing and Periapical X-rays (twice a year)
1 panoramic X-ray (every 3 years, beginning at age 6)
Free Sealant Repair for the duration of enrollment in the program
15% discount on all other dental services
THE FINE PRINT
Enrollment date begins on the first date of sign-up and provides coverage for 1 year (364 days) to the date. Enrollment is available on a yearly basis only.
Any costs associated with dental treatment are expected to be paid in full.
The exams, bitewing x-rays, cleanings, and fluoride must occur within the year of enrollment and cannot be carried over to the next year.
It is the account holder’s responsibility to make and keep appointments for his/her family members.
Our program is not transferable to another party or uncovered family member.
Participation cannot be combined with any other offer or dental plan.
All fees shall be based on our practice's UCR (usual, customary and reasonable) fees, which are subject to change, and not any other dental plan fees.
As this is an “In House” membership. Discounts cannot be applied to any specialist to whom we refer.
This plan is non-refundable and cannot be transferred.
If a cancellation in Wasatch Kids Loyalty Program is requested, the account holder is responsible for all discounts applied while under the plan.
Benefit coverage is effective for participants who remain enrolled in their plan at least until the completion of treatment. If enrollment expires before treatment is completed, coverage is no longer available.
